Use this checklist to determine if it's time for a no-charge assessment by a CareLink registered nurse. 1. Are there chronic health conditions present? Diabetes High blood pressure Arthritis Emphysema Obesity 2. Has there been the recent onset of an acute medical condition? Heart Disease Stroke Cancer Trauma or fracture 3. Have there been recent physical changes? Weight gain or loss Incontinence Balance problems Reduction in physical activity Sleep patterns 4. Is ther evidence of emotional and mental health issues? Anxiety Depression Dementia Psychosis 5. Have behavioral patterns changed? Eating habits Significant forgetfulness Wandering off Confusion Disorientation Sadness Appearance (cleanliness, neatness) 6. Have any of these issues and/or changes caused concerns about the current environment? Safety of the neighborhood Safety in the home Lack of close neighbors or family for support Availability of transportation 7. Have opportunities for and level of social interaction diminished?
