About CareLink

CareLink was founded in 1998 by two RN's and a health care consultant who saw a need for coordination of care for a variety of clients. They believed that nurses could improve results by coordinating, managing and, when appropriate, providing services directly to clients based on the individual client's needs.

CareLink believes that client needs are best met through careful and thorough assessment, individualized care plans, client-centered service delivery, outcomes measurement and continuous quality improvement. In addition to nursing services, CareLink provides supportive services such as assistance with personal care, light housekeeping and medication reminders. To insure continuous compliance with the highest standards of nursing care, CareLink is accredited by the Joint Commission. All CareLink nurses are appropriately licensed and insured.

Recognizing that nursing services must be cost effective to be available to all the individuals who can benefit from nursing assistance, Many CareLink services are affordably priced and CareLink assists clients in determining if coverage is available from the client's insurance plan.

CareLink continues under the ownership and direct management of its founders, who remain committed to providing a coordinated approach to health care for the elderly and the chronically ill. CareLink has just one goal – coordinating your health care needs so that the best possible outcome is achieved.
