Chronic illnesses such as diabetes, high blood pressure or asthma present many challenges for the patient and the family. Understanding the diagnosis, the treatment options, the medications prescribed by your physician, the lifestyle changes required... it can be overwhelming.

CareLink nurses help you manage your chronic condition so that you can enjoy the highest level of health possible. Upon referral by your physician, your insurance or upon direct contact by you, CareLink nurses can provide an initial assessment and develop a plan to help you understand your disease and how you can work with your physician to manage your situation. CareLink services include:

• Nursing assessment of your knowledge of your condition
• Implementation of customized education about your disease
• Treatment option review
• Treatment plan explanation
• Medications review and organization
• Coordination with other health care providers

In all cases, CareLink nurses act as care coordinators, bridging the gaps between the doctor's office, the hospital and other providers to help insure a coordinated approach.

CareLink nursing services may be covered by your insurance, and we will assist you in determining if coverage is available from your plan. CareLink nurses are licensed, insured and accredited.
