CareLink Care Coordination

Sometimes the health care system can be pretty confusing. It takes a lot of time and knowledge to coordinate all the services you might need, particularly for people with serious illnesses or ongoing acute health care needs. Primary care physicians, specialists, home care, medical equipment and supplies, prescriptions, rehabilitation... it helps to have a knowledgeable care coordinator on the case.

CareLink nurses focus on the needs of the patient, to bridge the gaps between all the different services and providers you may need. The result is better use of resources, faster recovery and greater peace of mind for patient, physicians and families. Upon referral by your physician, your insurance or upon direct contact by you, CareLink nurses can provide an initial assessment and develop a plan to help make sure the health care system works for you. CareLink services include:

• Coordination with all the physicians and other providers involved in your case
• Interface with your insurance administrator and/or managed care provider
• Ongoing contact with case manager
• Direct nursing services as appropriate

CareLink nursing services may be covered by your insurance, and we will assist you in determining if coverage is available from your plan. CareLink nurses are licensed, insured and accredited.
