This is the only situation where I have had nurses regularly accompany patients to their visits. I am aware that they help set up medicine, draw blood, do wound care, provide education, and do a lot of coordinating, coming to doctors appointments, and other services. They are a great help to patients and to their families.

When you or a member of your family needs nursing care outside the hospital or physician office, CareLink, Inc. is available to meet your needs. CareLink's registered nurses can provide the continuing nursing care you need to complete your recovery following hospitalization. And, CareLink can provide nursing care in those situations when treatment can be delivered in the home or at the worksite, to avoid a hospitalization or trip to the doctor's office.

CareLink nurses provide a variety of services, always with the goal of achieving the best possible outcome for the client and family. Upon referral by the physician or hospital, or upon direct contact by you, CareLink provides an initial assessment and develops a plan of care in conjunction with your physician. All direct nursing services are provided under the physician's orders. CareLink services include:

• IV therapy
• Dressing changes
• Blood draws
• Patient teaching
• Medications organization and administration
• Wound care

In all cases, CareLink nurses act as care coordinators, bridging the gaps between the doctor's office, the hospital and other providers to help insure a coordinated approach.

CareLink nursing services are usually covered by your insurance, and we will assist you in determining if coverage is available from your plan. CareLink nurses are licensed and insured.
