CareLink provides a variety of health services to help clients achieve their best possible levels of health. Through supportive services, direct nursing service, and care coordination we help the elderly and the chronically ill of all ages stay safe and healthy.

By focusing our attention on the needs of the client, we can bridge the gaps between the doctor's office, the hospital, worksite and home to help insure a coordinated approach to health treatment and management. That means better use of resources, faster recoveries and greater peace of mind for physicians, patients and families.

CareLink nurses are experienced in various aspects of nursing, including case management and home care. CareLink nurses have years of experience in patient education, hospital nursing, intensive care, IV therapy, physician office nursing and industrial nursing. Strict confidentiality applies in all settings. All services are provided through individualized care plans and under physician direction as appropriate. CareLink is accredited by the Joint Commission in both offices.
